Online Competition Entry Application

This is a two-part process, details listed below. If you are unable to complete both parts prior to the zone entry deadline please use the traditional method of entry whereby you sign up within your curling club. Your OCA Club Representative can then handle the entry on your behalf. Please DO NOT enter via both methods. Online Process - Part 1 Complete the online entry application. Be prepared to supply: a valid email address, submit credit card information and Name all four players and coach if required You will be issued a confirmation number after you complete and submit the form. Online Process - Part 2 You will be emailed a confirmation form requiring the signature of your OCA Club Representative or Club Manager. If you do not receive this email please check your spam filtering prior to calling the OCA Office at 905-831-1757 or 1-877-668-2875. Do one of the following: Print the form, have it signed, fax the form to the OCA Office at 905-831-1083 OR Forward the email to your OCA Club Representative or Club Manager asking them to forward it (with any comments) to the OCA Office at entries@ontcurl.com Important Notice Online entries must be submitted by the deadline date. If you fail to complete both steps of this process your team will not be included in the draw and your entry fee defaulted. If you need to withdraw your entry prior to the entry deadline, please e-mail the OCA office quoting the confirmation number and the reason for withdrawing. All competition and eligibility rules will apply, as per the competition sign up sheets and the current OCA rules supplement.
